For anyone who has lost their certified nursing assistant certificate, you can still get a replacement card if you know what to do. There may sometimes be a fee that is associated with this, but you can easily do it online or through the postal service. You will need to get a hold of the Board of Nursing to help better determine what you will need to do or need in order to receive your replacement from and to get you in touch with them. There are some states that are going to require you to fill out a replacement form online while others are going to ask that you do the replacement through the regular snail mail, or postal mail.

If you are given the option of replacing your CNA license online, you should go with this option as it often times gets processed faster. You will be given directions on where to find the replacement forms and how to do everything online. There is also the option to print out a PDF form and mail it in with your replacement fee in the form of a check or money order. Each state may differ as far as the replacement fee, but on average, you will be paying $25 per license you want replaced.


You might even be able to do it over the phone and talk with a representative at the Board of Nursing located in your state which you were licensed in. You can either call or email them, and if all else fails, you can write a letter to the Board of Nursing and request a replacement. You will need to provide your SSN, date of birth, a money order for the set amount, and also your CNA license number. If you cannot remember this, you are going to have to talk to someone and they will provide it to you. Based on the method you chose to request your replacement license, you are going to have to wait as much as 8 weeks, and most online replacements are usually processed and received sooner than that.

Once you receive your new nursing assistant license, be sure that you make a few copies and keep them in a place where you will be able to find them in case you lose yours again. This avoids the added fees and the long process again. You should ideally keep the original in a safe place and use your copies when needed.


If at any time you have to change either your name or address, inform the Board of Nursing as soon as you possibly can. This is to ensure that your records are kept up to date and that you always have the right information on file should you need to renew or replace your license. You should notify them within 30 days, and this also allows them to have the right information should they need to mail renewal information to you.
